WORLD DOWN SYNDROME DAY SADI E MI LA and Meet

By, Pam Teel March 21 st is World Down syndrome Day, (WDSD), which is a global awareness day officially observed by the United Nations since 2012. The date for WDSD - the 21 st day of the 3rd month, was selected to signi fy the uniqueness of the triplication (trisomy) of the 21 st chromosome, which causes Down syndrome. Down syndrome (or Trisomy 21) is a condition in which a person has an extra chromo some. Chromosomes are “packages” of genes in the body. They determine how a baby’s body forms and functions as it grows. Around 1 in every 800 babies will be born with Down syn drome. Down syndrome occurs naturally: there is no known cause. It usually causes varying degrees of intellectual and physical disability and associated medical issues. Most of us have 23 pairs of 2 chromosomes for a total of 46. Those with Down syndrome have an extra copy of the 21st chromosome. (Thus, the name Trisomy 21) Down syndrome affects every gender, race, nationality, and socioeconomic status. There are approximately 6000 babies born every year with Down syndrome. Services to help people with Down syndrome have changed dramatically in the past half

ROLLO FAMILY WITH SADIE

century. In the 1960s, when a baby was born with Down syndrome, one of the only options was to institutionalize a child. Luckily around the early 1970s, things changed for the better. More emphasis went into programs that focused on education and treating other related health issues. There’s no way to cure Down syndrome because it’s a genetic condition, but there are many ways to help people living with the condition manage their disabilities and lead happy and productive lives. It’s not as much about treating Down syndrome, but more about finding therapies and services that make a positive impact. It really comes down to the in dividual, and treatments should be based on what each person needs. For children with Down syndrome, early intervention should start as soon as possible. Under federal law, states must offer free, appropriate public education for all children with (Ds) as part of the Individuals With Disabilities Education Act, for all children with Down syndrome. The law includes coverage of Early Intervention services designed to help address the developmental delays children with Down syndrome face. Such services are available and should be started any time after birth until the child turns 3, or until the child is eligible to enter kindergarten. If you are looking for information on this, check out the National Early Childhood Technical Assistance Center’s helpful list of resources for families. These programs provide children with Down syndrome the various types of therapies they need and that have been proven beneficial, including occupational therapy, physical therapy, and speech therapy. Early intervention will definitely dictate the trajectory for your child. Becca and Andrew Rollo, from Millstone Township, received a diagnosis quite early that their fourth child had Down syndrome. It gave them plenty of time to experience a wide range of feelings on waiting for her delivery. They wondered if they would ever have a chance to go on vacations or take day trips with a child with Down syndrome, or even function as a typical family. Sadie was born May 26, 2020 at CentraState Hospital during the pandemic. Having a baby with a disability during a pandemic actually had a lot of silver linings for the couple because it gave them so much family time doting on the new baby, and a chance to get to know Sadie in a judgment free zone before stepping out into the great, wide world as her advocates. The Rollo’s felt blessed to have found invaluable resources that helped take that fear out of them. One such resource, DSDN (Down syndrome Diagnosis Network), runs support groups for parents through Facebook. “There is a private pregnancy support group, and once your baby is born, they group you into a birth group with other babies born the same calendar year as your baby. Sadie has quite the epic crew of “homies with extra chromies” and we have found an amazing community in the “Lucky Few.” They often say it’s the best club you never would have thought to join. There is a shared experience instantly that bonds families in the Ds community that feels very special. We would be lost without our local group, Network 21 serving Monmouth & Ocean counties. Having others who have gone before and paved the way gives a family new to the journey so much hope and reassurance. While on this journey, I was in spired with some other “lucky mamas” to collect funds to send some “sunshine” to other disability families in an hour of need. We have formally become a 501(c)3, 321 Sunshine. You can find out more at 321Sunshine.org.,” stated Sadie’s mother Becca. There is even a platform on Instagram, started by one of Becca’s friends called: @happinessisdownsyndrome, which has advocacy videos for moms with new diagnosis. “Sadie’s smile lights up a room and she loves to say hi to anyone and everyone. She is known to request hugs and go into the arms of complete strangers and charms them in an instant. No, she’s not always happy- she feels all the feelings the same way the rest of us do, but it does feel like that extra chromosome gave her an extra dose of love to share with the world,” Becca noted. Sign language has been very helpful for Sadie to bridge the gap in communication. People with Down syndrome tend to be visual learners, so she picks up on signs quickly. It also alleviates some difficulty caused by having low muscle tone, which extends to the tongue, which has 8 muscles. As Sadie adds spoken words to her vocabulary, the ASL signs helps to understand her speech by differentiating between words she may say similarly. She has a Signing Time sub scription and according to Becca, the videos by Rachel Coleman have taught her so much at such a young age.

Teddi has been a member of the Harish family since early 2018 after a dance competition bet left the Har

ish’s Siri per-

with the promise of a new pet.

Harish, a first-year at the University of Connecticut (UConn), has been a dancer for most of her life, often forming with her younger sister and most recently, becoming a member of Husky Bhangra, a competitive dance team at UConn. While dance has been an opportunity for her to connect with other people, she said it was also the reason the Harish's welcomed Teddi, a Shih Tzu Yorkie puppy, to their family in January of 2018. “My sister really, really wanted a dog,” she said. “She eventually convinced my mom that if we won a dance competition that my mom would get us a dog, and we ended up winning.” Even though Harish was reluctant about bringing in another pet following the passing of previous family pets, and her parents were initially against the idea of owning a dog due to religious beliefs, the Harish’s soon began their search for a puppy. cause of her mother’s fear of large dogs, the family was keen on getting a small dog that was more practical said they looked at local shelters such as The Humane Society and Sammy's Hope, but later connected with a breeder from New York where they quickly bonded with Teddi, who was just a couple of weeks shy from turning 3 months old at the time. “We actually brought her home that night, too,” Harish said. look after.

She said Teddi was fearful when her family arrived back to New Jersey from New York as it was Teddi’s first time alone without her two brothers by her side. Though it took some time for Teddi to become comfortable in her new home and for the Harish’s to become adjusted as well, Teddi now enjoys seeing new people and has made the family complete. “Honestly, I think my mom…and my dog have become so close over the years. You can’t separate them at all,” Harish said. “Whenever [Teddi’s] sick…my mom is the first person she goes to for comfort.” In fact, Teddi joins the family for walks around the community, movie nights, hikes at Thompson Park and games of hide-and-go-seek. Teddi also partakes in the tradition of wearing an Indian dress, presenting food to be blessed during religious ceremonies and celebrating different cultural festivals. Typically, Teddi’s routine consists of going on a walk around the outside of the house as well as surrounding areas, having kibble with chicken gravy for breakfast, practicing tricks, taking a bath, eating a snack of either a biscuit or an egg and having her paws cleaned depending on the weather. Despite attending school in a different state and no longer seeing Teddi everyday, Harish said she looks forward to spending time with Teddi when she visits her family for the holidays. “Taking care of a pet…also reminds you to take care of yourself,” she said. “You put that love out there and you get so much love back, and it’s just the best feeling ever.”

WHAT IS SLEEP DISTURBANCE? By, Lauren Kowlacki

Sleep allows your body to rest, recover, and reset for the next day. When sleep is interrupted or you are not getting enough sleep, it can impact your physical and mental health, as well as your day-to-day life. Disturbances in sleep is a common symptom and can be caused by physical changes caused by a brain or spine tumor, side effects from treatment, or cancer related anxiety. There are five major types of sleep disorders that affect normal sleep: 1. Insomnia: being unable to fall asleep and stay asleep 2. Sleep apnea: a breathing disorder in which breathing stops for 10 seconds or more during sleep 3. Hypersomnia: being unable to stay awake during the day 4. Circadian rhythm disorders: problems with the sleep-wake cycle, making you unable to sleep and wake at the right times 5. Parasomnia: acting in unusual ways while falling asleep, sleeping or waking from sleep, such as walking, talking, or eating Ways to Manage Sleep Disturbance • Keep a log of your sleep (include your naps and amount of time you sleep at night), sleep disturbances, and what you are doing to manage it daily using a journal. • Good sleep hygiene is important for improving the quality of your sleep. • Have a dark and quiet sleep environment with a comfortable temperature. • Use your bed for sleeping and intimacy only, not for watching TV or working. • Stick to a sleep schedule so you so you go to bed and wake up around the same time every day. • If you do not fall asleep within 20 minutes, try a relaxing activity or getting out of bed and returning when you feel sleepy. • Avoid naps longer than one hour and naps close to bedtime. If possible, nap somewhere other than your bed. • Relaxation strategies can help reduce anxiety and improve your sleep.

• Mindfulness-based stress reduction helps you recognize your current emotions, thoughts, and bodily feelings to better deal with stressful experiences. These strategies include yoga and meditation. • Other soothing activities can include reading or listening to music. • Ask your doctor about cognitive behavioral therapy. Some self-care activities you can do to improve your pain can include diet, exercise, sleep, or medication.

• Try to follow the diet your doctor recommends. Limit processed foods, which are pre-packaged and contain added sugars and fat. Eat plenty of fruits and vegetables. Eat a high-protein snack two hours before bedtime and six small meals throughout the day. Drink plenty of water to stay hydrated. Avoid drinking fluids and eating heavy meals before bedtime. Avoid alcohol, caffeine and sugary drinks. • Avoid exercising within three hours of bedtime as it can lead to interrupted sleep. • Corticosteroids and other medications may interfere with sleep when taken later in the day. Talk with your doctor about adjusting your medication schedule.

KNEE PAIN Dr. Scott Paris

QUESTION: What’s the best way to treat knee arthritis? Consider a non-surgical treatment that really works!

O s teoarthritis of the knee is the #1 leading cause of disability in the U.S. Over 27 million people suffer with knee arthritis and that number continues to grow. With knee arthritis, the cartilage breaks down and wears away, leaving the bones to rub together causing pain swelling stiffness and limited range of motion. As the cartilage breaks down, the knee loses its lubricating fluid. We found that by replacing this lubricating fluid with a natural substance called Hyaluronate, the lubrication is restored and many patients experience amazing pain relief.

DOES IT HURT? Many of our patients report that they feel little to no pain at all! We do everything possible to minimize any pain from the procedure, from pain relieving spray and a local anesthetic, to precision guidance that ensures we are always in the right spot. WILL MY INSURANCE COVER HYALURONATE? This procedure is covered by most major insurance companies including Medicare. Our insurance department pre-verifies every patient. WHO WILL BE PERFORMING THE HYALURONATE PROCEDURE? Dr. Scot Paris is the medical physician who performs the Hyaluronate injection. Dr. Paris is trained as a general surgeon and has extensive experience with a variety of surgical procedures. With his conservative approach, he has excelled in the non-surgical treatment of knee pain with the use of fluoroscopy guided Hyaluronate injections. SO HOW DO I KNOW IF YOUR KNEE PAIN TREATMENT PROGRAM WILL HELP ME? If you are tired of suffering with painful arthritis of the knee, and you want to see if the latest advances in non-surgical treatment can help, all you have to do is call 732-683-1800 to set up a risk free consultation. During the consultation

WHAT IS HYALURONATE AND HOW DOES IT WORK? Hyaluronate is a natural lubricating fluid that acts as a lubricant and shock absorber inside the knee joint. The substance in Hyaluronate is normally found in high levels in healthy joints. Replacing the hyularonic acid with Hyaluronate lubricates the joint and reduces pain. Hyaluronate has been administered over 40 million times and has been proven to provide long-lasting relief of knee pain from osteoarthritis. WHAT IF YOU ALREADY TRIED KNEE INJECTIONS? If you have had injections like this in the past and experienced poor results, there is still hope! Many doctors in this area offer similar injections, but very few use precision fluoroscopy guidance. Fluoroscopy guidance ensures that the pain relieving Hyaluronate reaches the knee capsule where it bonds with the joint fluid to create a lubricating and cushioning layer. Without fluoroscopy, it is estimated that doctors “miss” the capsule over 30% of the time resulting in poor results. We want each and every one of our patients to have the greatest chance of success, so every injection is done with precision fluoroscopy guidance to ensure that the Hyaluronate gets into the capsule 100% of the time. Because of this, patients who previously felt little to no relief with non-flouroscopy guided injection are now getting excellent results. The other “big difference” in our results is the multitude of other supportive care options that we have available to enhance the effectiveness of the Hyaluronate procedure. These options include state of the art medical laser treatment, physical therapy, manipulation, corrective osteoarthritis bracing (takes pressure off the damaged section of the knee), and natural anti-inflammatory products. The combination of these treatments offers you what we feel is the most comprehensive non-surgical knee treatment program available.

all of your questions will be answered in a warm and friendly environment. Knee replacement surgery is a very extreme measure to take without considering all of your other options first. Once it’s done there is no turning back. For that reason we encourage you to schedule an appointment to see if you are a candidate for Hyaluronate treatment.

We are proud to offer our patients a comprehensive knee pain treatment program that includes the best non surgical methods to relieve pain all under one roof. We look forward to having the opportunity to help you feel better. Take the first step in ending your knee pain and call to schedule an appointment today!

Dr. Scott Paris using precision fluoroscopy guidance to ensure the best results possible.

Avoid Being Pulled Over by The Police By John Bazzurro Obviously, the easy way to avoid being pulled over is to refrain from violating any traffic violations. However, in my practice, I have recently seen a number of people pulled over for reasons other than moving violations under the traffic code. Because the first defense for attorneys in representing individuals in municipal court is to challenge whether or not the officer had appropriate cause under New Jersey law to actually make the stop in the first place, any “legal” reason that an officer has to effectuate a traffic stop makes the attorneys’ defense of any case much more difficult. Other than moving violations, the four most used violations by officers to validate a potential, otherwise “illegal” traffic stop have to do with simple equipment violations that can be resolved rather quickly and inexpensively. The first equipment violation relates to the “improper” covering of a license plate by a license plate frame. License plate frames are generally the plastic “frames” that are given out by most car dealers or that can be bought in any automotive store. New Jersey law prohibits a motor vehicle from having a license plate frame which conceals or otherwise obscures “any marking” on the license plate. Thus, if a portion of the words “New Jersey” or “Garden State” is obscured on your license plate in any way, a police officer has the legal right to effectuate a traffic stop. The second violation relates to items hanging off of your rearview mirror and other obstructions of the windshield and front side windows. New Jersey law prohibits the operation of any vehicle which is “constructed, equipped or loaded” so as to “unduly” interfere with the driver’s vision. New Jersey case law has held that items hanging from the rearview mirror could fall under the language of this statute. However, the statute also includes the existence of stickers, signs, radar detectors, navigation systems or any other device or mechanism attached to the front windshield. The third violation relates to tinted windows. The same statute that restricts the hanging of anything from the rear view mirror also has been construed to hold that tinted windows constitute such an obstruction of view and, as such, the tinting of any three of your front windows would constitute appropriate cause for a law enforcement officer to pull you over even where you have not committed a moving traffic violation. The fourth violation relates to broken lights on your vehicle such as headlights, taillights, brake lights and signal lights. Even where you have not committed a moving traffic violation, a law enforcement officer has appropriate cause to effectuate a traffic stop where he observes that one of your lights is not operational. It should be noted that the fines and penalties for these violations, in and of themselves, are not excessive. However, as is often the case, the law enforcement officers, upon effectuating a traffic stop will then observe that the driver or passengers in the vehicle have violated other motor vehicle and/or criminal statutes such as driving while intoxicated (DWI), driving under the influence (DUI), possession of controlled dangerous substances (CDS), possession of drug paraphernalia, open containers of alcohol in a vehicle, possession of a weapon or any other offenses which the officer may observe during a traffic stop. Once you are pulled over for a valid reason, it is difficult for an attorney to challenge the traffic stop on the basis of articulable suspicion or probable cause. Accordingly, in light of all of the above, it is prudent to make sure that your motor vehicle complies with all of the above statutes in order to prevent being pulled over by law enforcement even where you have not actually committed a moving traffic violation.
