What is Sleep Disturbance? By, Lauren Kowlacki

Sleep allows your body to rest, recover, and reset for the next day. When sleep is interrupted or you are not getting enough sleep, it can impact your physical and mental health, as well as your day-to-day life. Disturbances in sleep is a common symptom and can be caused by physical changes caused by a brain or spine tumor, side effects from treat ment, or cancer-related anxiety. There are five major types of sleep disorders that affect normal sleep: 1. Insomnia: being unable to fall asleep and stay asleep 2. Sleep apnea: a breathing disorder in which breathing stops for 10 seconds or more during sleep 3. Hypersomnia: being unable to stay awake during the day 4. Circadian rhythm disorders: problems with the sleep wake cycle, making you unable to sleep and wake at the right times 5. Parasomnia: acting in unusual ways while falling asleep, sleeping or waking from sleep, such as walk ing, talking, or eating Ways to Manage Sleep Disturbance • Keep a log of your sleep (include your naps and amount of time you sleep at night), sleep disturbanc es, and what you are doing to manage it daily using a journal. • Good sleep hygiene is important for improving the quality of your sleep. • Have a dark and quiet sleep environment with a comfortable temperature. • Use your bed for sleeping and intimacy only, not for watching TV or working.

• Stick to a sleep schedule so you so you go to bed and wake up around the same time every day. • If you do not fall asleep within 20 minutes, try a relaxing activity or getting out of bed and returning when you feel sleepy. • Avoid naps longer than one hour and naps close to bedtime. If possible, nap somewhere other than your bed. • Relaxation strategies can help reduce anxiety and improve your sleep. • Mindfulness-based stress reduction helps you recognize your current emotions, thoughts, and bodily feelings to better deal with stressful experiences. These strategies include yoga and meditation. • Other soothing activities can include reading or listening to music. • Ask your doctor about cognitive behavioral therapy. Some self-care activities you can do to improve your pain can include diet, exercise, sleep, or medication. • Try to follow the diet your doctor recommends. Limit processed foods, which are pre-packaged and contain added sugars and fat. Eat plenty of fruits and vegetables. Eat a high-protein snack two hours before bedtime and six small meals throughout the day. Drink plenty of water to stay hydrated. Avoid drinking fluids and eating heavy meals before bedtime. Avoid alcohol, caffeine and sugary drinks. • Avoid exercising within three hours of bedtime as it can lead to interrupted sleep. • Corticosteroids and other medications may interfere with sleep when taken later in the day. Talk with your doctor about adjusting your medication schedule.

By, Lauren Kowlacki Because the nicotine in tobacco is highly addictive, people who quit may experience nicotine withdrawal symptoms, es pecially if they have smoked or used other tobacco products heavily for many years. Although many of the examples in this fact sheet refer to smoking, the tips are relevant for those who are quitting the use of any tobacco product. Common nicotine withdrawal symptoms include: • nicotine cravings • anger, frustration, and irritability • difficulty concentrating • insomnia • restlessness • anxiety • depression • hunger or increased appetite Other, less common nicotine withdrawal symptoms include headaches, fatigue, dizziness, coughing, mouth ulcers, and constipa tion. The good news is that there is much you can do to reduce nicotine cravings and manage common withdrawal symptoms. Also, it may help to know that nicotine withdrawal symptoms do subside over time. They are usually worst during the first week after quitting, peaking during the first 3 days. From that point on, the intensity of symptoms usually drops over the first month. However, everyone is different, and some people have withdrawal symptoms for several months after quitting. What can I do about nicotine cravings after I quit? People who use tobacco products get used to having a certain level of nicotine in their body. After you quit, cravings develop when your body wants nicotine. This may occur long after your body is no longer addicted to nicotine. In addition to this physical craving, you may experience a psychological craving to use a tobacco product when you see people smoking or are around other triggers. Your mood may change when you have cravings, and your heart rate and blood pressure may go up. The urge to smoke will come and go. You may start experiencing cravings within an hour or two after your last use of tobacco, and you may have them frequently for the next few days or weeks. As time passes, the cravings will get farther apart. However, you may have occasional mild cravings months or years after you quit. Here are some tips for managing cravings: 1. Try nicotine replacement products or ask your doctor about other medications. 2. Remind yourself that cravings will pass. 3. Avoid both cravings and situations that you used to associate with using tobacco products. 4. As a substitute for smoking, try chewing on carrots, pickles, apples, celery, sugarless gum, or hard candy. Keeping your mouth busy may stop the psychological need to smoke. 5. Try this exercise: Take a deep breath through your nose and blow out slowly through your mouth. Repeat 10 times. Identity Theft in Business Is Real: How to Avoid Tax-Related Identity Theft? By, Erin Mumby

Tax-related identity theft is promi nent on the IRS’s Dirty Dozen list of tax-scams. Tax-related identity theft is not limited to stealing personal infor mation of individuals. Because of suc cessful efforts to crack down on such

identity theft, thieves have shifted their focus to businesses. They create and use, or attempt to use, the identifying information of businesses to obtain tax benefits. For example, according to the IRS, cybercriminals that obtain a business’s tax identification number may file a return claiming a tax refund because of a fuel credit or a research credit used as a Social Security tax offset. It’s essential for you as a business owner to understand that tax-related identity theft is real and could hit your company. THE IRS lists 5 signs of identity theft: 1. The IRS rejects an e-filed return saying it already has one with that employer identification number (EIN) or Social Secu rity number (SSN). 2. The IRS rejects an extension to file request (Form 7004 for entities; Form 4868 for Schedule C or F filers) saying it already has a return with that EIN or SSN. 3. The business receives an unexpected tax transcript. 4. The business receives an IRS notice that doesn’t relate to anything they submitted. 5. The business doesn’t receive expected or routine mailings from the IRS. Identity thieves are continually creating new scams to utilize business information to their financial advantage. Here are some examples of previous scams to look out for: • FormW-2/SSN data. Cyber thieves use spoofing techniques to make an email appear as if it is from a company executive. The email is sent to an employee in the payroll or human resources departments, requesting a list of all employees and their Forms W-2. This scam, referred to as business email compromise (BEC) or business email spoofing (BES), enables criminals to use employees’ personal information to commit tax fraud and financial theft. If your business data is compro mised, tell employees and alert the IRS at www.dataloss@irs.gov. • Direct deposit changes. An email from cybercriminal posing as an employee asks the payroll or HR department to change his/her direct deposit of salary or wages. A new bank account is provided, which is used to siphon off an employee’s com pensation. Big tipoffs that emails are from scammers are poor grammar and misspellings. The IRS has provided this example of a wire transfer scam: “Please confirm the receipt of my message, Authorized can you handle domestic transfer payment now?” Protection from tax-related identity theft should be part of your overall best practices for data protection from hackers and scammers. It is important to use basic security measures and work with good tax pros. Be sure your CPA or other tax profession al is safeguarding your tax information.

DO YOU NEED A LAWYER IN MUNICIPAL COURT? BY JOHN BAZZURRO As indicated on our website, there are numerous charges that are heard in the Municipal Courts in the State of New Jersey. These range from low level motor vehicle violations such as going through a stop sign, careless driving, failure to maintain your lane and other charges that generally carry very low fines and two or less motor vehicle points. If you have been charged with any motor vehicle violation that carries more than two motor vehicle points upon a conviction, we suggest that you obtain the advice of counsel before going to court. The purpose of this article is to provide the reader with some information as to whether or not they need legal representation in Municipal Court on a low-level motor vehicle violation that carries two or less motor vehicle points. As most citizens of New Jersey know, municipal prosecutors will, generally, as a matter of course offer a “no point” or “zero point” violation as part of a plea bargain if you are charged with one of these low-level motor vehicle violations. The monetary cost of these “no point” or “zero point” violations are rather excessive in that you are required to pay a $250.00 surcharge for this “privilege.” The statute which provides this “no point” plea is called the unsafe driving statute. The basic theory in accepting a “no point” or “zero point” plea bargain is to potentially avoid motor vehicle points on your license in order to keep your auto insurance premiums lower and, potentially, to avoid the suspension of your driving privileges where you are in danger of accumulating 12 motor vehicle points which will result in a temporary suspension of driving privileges. However, it should be pointed out that there are a number of situations in which your acceptance of such a “no point” or “zero point” plea bargain may adversely affect your rights without your knowledge. One such situation is where you have already previously pled guilty to unsafe driving on more than one occasion. Your third guilty plea to unsafe driving within a certain period of time will subject you to four motor vehicle points when you may be under the mistaken belief that you will not be receiving any motor vehicle points. Another such situation where you potentially will need legal representation is where the low-level motor vehicle violation is the result of a motor vehicle accident. The bottom line is that, regardless of what type of motor vehicle violation you are charged with, you should seek the advice of an attorney before appearing in Municipal Court by yourself and entering into a plea bargain which may subject you to other legal ramifications of which you were not aware. Typically, when we receive a call for a motor vehicle violation, we initially attempt to obtain as much information as possible from the potential client to determine whether or not they will require legal representation given their individual circumstances. Accordingly, please feel free to contact us to discuss your individual situation to determine whether or not legal representation is required.

Hot Shots: New Injectable Treatment Options for Arthritis Pain By Mutahir Abidi, MD

There’s good news for the more than 54 million Americans who have been diagnosed with some form of arthritis: a new class of medications are providing quick, safe relief from pain and swelling. Types of Arthritis There are more than 100 types of arthritic conditions. Symptoms include inflammation and swelling of the joints, which causes pain, discomfort, and movement issues. One of the biggest misconceptions about arthritis is that it only affects seniors, but two-thirds of ar thritis patients are younger than 65 years of age. It can even affect children. As a rheuma tologist, we are trained to diagnose and treat every type of arthritis in patients of all ages. There are two types of arthritis: • Inflammatory arthritis typically af fects younger patients. It occurs when the body’s immune system attacks the cells in the joints, which in turn causes severe joint pain, stiffness and swelling. Some of the inflammatory arthritic conditions include rheuma toid arthritis, arthritis related to pso riasis, Crohn’s disease, ulcerative coli tis, lupus, or other conditions. • Non-inflammatory arthritis primarily affects older patients. The most com mon type is osteoarthritis, a degen erative joint disease that can develop as we age and the cartilage within our joints begins to break down. Carti lage is the tissue that covers the ends of bones in a joint. It allows bones to glide over one another and acts as a “shock absorber.” A healthy knee joint also contains a small amount of syno vial fluid, a thick, gel-like substance that cushions the joint and provides lubrication to reduce friction. As we age, this lubrication decreases. Treating Inflammatory Arthritis For inflammatory arthritis, treatment is focused on stopping the immune system from attacking the joints. Because each per son’s body responds to treatment different ly, there are two classes of medications that we use: Disease Modifying Anti Rheumatic Drugs (DMARD) and biologic agents. An example of DMARD is methotrexate. Methotrexate was introduced as treatment for rheumatoid arthritis about 30 years ago and replaced high-dose aspirin and non-ste roidal anti-inflammatory drugs (NSAIDS) as the treatment of choice. Instead of masking pain, methotrexate gets to the root of the is sue to control the source of inflammation. It treats severe psoriasis by slowing the growth of skin cells to stop scales from forming and

treats rheumatoid arthritis by decreasing the activity of the immune system. A new field of medications called biolog ics have revolutionized the way we treat ar thritis. Before, people who had rheumatoid arthritis that didn’t respond to other treat ments could become crippled and wheel chair-bound. Now, within a day or two of receiving a biologic agent, patients are feel ing remarkably less pain and better mobility, with limited side effects. Depending on the medication – and there are more than 10 bi ologic medication options – it can be given as an injection, a pill, or an intravenous in fusion. Biologics are particularly helpful for patients with rheumatoid arthritis, psoriatic arthritis, and spondylitis. Some examples of biologic medications include Enbrel, Humi ra, Orencia, Remicade, and Xeljanz. Treating Non-Inflammatory Arthritis For patients with non-inflammatory ar thritis like osteoarthritis, the prevailing treat ment had been aspirin until non-steroidal anti-inflammatory drugs (NSAIDS) were developed 50 years ago. This class of medi cations include Motrin, Advil, naproxen, and ibuprofen. Local joint injections like cortisone in jections have been used to reduce joint pain. Unfortunately, some patients had a bad expe rience with shots because either not enough medication was used, shots were spread out too much to be effective, or, in some extreme cases, the provider hit a bone while adminis tering the injection. It’s important that these injections are given by a trained rheumatol ogist using ultrasound imaging to guide the needle into the joint. Ultrasound enables physicians to precisely guide the shot into the joint, increasing the effectiveness of the medication. It also can find fluid in the joint that can cause the joint to swell and dilute the medication. In these cases, fluid can be drained before the injection to ensure that treatment is effective. If you’ve had a corti sone injection before and had a bad experi ence, try it again with a physician who uses ultrasound for the best results. In fact, in jections done “blindly” without ultrasound guidance can miss the mark 30 percent of the time. Another option for osteoarthritis is vis cosupplementation injections. During vis cosupplementation, a thick fluid called hyaluronate is injected into the knee joint, improving the lubricating properties of the synovial fluid. The procedure can reduce the pain from osteoarthritis and improve mobil ity. It’s often paired with cortisone injections to boost effectiveness.

Another new option is Zilretta, an extend ed-release corticosteroid that slowly and con tinuously releases medicine into your knee for about six months. It works better and longer than traditional cortisone and doesn’t affect a patient’s blood sugar, an important benefit for diabetics. Over-the-Counter Options Some people prefer to treat joint pain with more natural, over-the-counter treatments. While they may not provide the level of treat ment necessary for everyone, for the most part, they aren’t harmful, so I don’t discour age my patients from taking them. Some options include: • Glucosamine and chondroitin: Fre quently paired together, glucosamine and chondroitin may provide some relief. However, clinical studies are inconclusive about their effectiveness. Look for formulations with 1,500 mg of glucosamine and 1,200 mg of chon droitin. • Turmeric: Studies also are mixed re garding turmeric, but many of my patients report that they have less pain while taking it. Look for a con centrated formulation with curcumin and black pepper (piperine), which increases the body’s ability to absorb turmeric. Now more than ever, rheumatologists have an array of highly specialized medica tion options to treat all types of arthritis – and change patients’ lives. Our goal is to im prove patients’ quality of life without opioids or surgery unless other treatments fail. Each patient should receive a comprehensive eval uation and a customized treatment plan that takes into account age, lifestyle, level of activ ity, other medical issues, and support system. Most treatment plans will include a physical therapy component to increase functionality and movement. Also, it’s important to look at a patient’s total body, not just the specific joint that’s in pain. For example, if you have pain in your knee, since all of your bones are connected, it could be caused by an issue with your back, feet, or ankles, so it’s import ant to rule out other issues first. There are a lot of new and promising op tions for treating arthritis, alleviating pain, and improving quality of life. There’s no rea son to continue to live with pain. Megalomania is a narcissistic personality disorder in which some one is excessively driven by money, power, and strength. The name of the condition, which comes from the Greek language, means “big obsession.” Generally, a megalomaniac thinks highly of themselves and has an unrealistic perception of their abilities. An individual who suf fers from megalomania is also likely to suffer from other personality disorders. Genetics, brain damage, stress, drug abuse, childhood trauma, alcohol addiction, and other mental disorders can contribute to a megalomania diagnosis, although researchers have not yet deter mined a single cause leading to the condition. Symptoms of the disease include immense pride and an intense need for power, overwhelming feelings of sadness, narcissism, mood swings, aggressive behavior, lack of empathy, exploiting others with out remorse, high self-esteem and focusing on one’s own opinions while dismissing everyone’s around them, among other things. Diagnosing the disorder typically includes looking at the patient’s medical history and talking to close relatives and friends of the pa tient to better understand their symptoms, if any. Treatment for megalomania almost always includes prescription medication and sometimes, psychotherapy. Psychotherapy regard ing treating megalomania entails confronting the patient’s narcis sism by talking to a therapist. While it is possible to have someone with megalomania in your life, some cases of the disorder can be dangerous. Researchers rec ommend keeping one’s distance from these patients, being aware that they will likely not return the effort you are putting in the rela tionship and refraining from trying to change their behavior. However, all the different products in a beauty regime can get costly. Worldwide, women spend about $426 billion on beauty products annually. The average woman spends approximately $15,000 on beauty products in her life. The cost doesn’t have to be this high if you love cosmetics! You can find items at lower prices or even avoid certain products that are unnecessary. Here are a few secrets that can help The difference between some high-end and drugstore brands can be just the pack aging. Beauty corporations often use the same product and package it differently. They tailor it to look fancy for luxury brands or less expensive brands for drugstore brands. 2. Anti-aging products won’t remove your wrinkles Most products designed to help erase signs of aging are a marketing ploy. Studies show that if anti-aging products don’t contain retinol they won’t work. 3. Sunscreen is the only true anti-aging product The only way to prevent aging, is to prevent wrinkles by applying sunscreen. Only 1/10 people use sunscreen regularly! 4. Avoid aerosols as much as you can Aerosol sprays have poisonous fumes that you could be inhaling into your lungs. Hold onto your dry mascara As long as your mascara hasn’t expired, you can save it for a few more uses. If your favorite tube is drying up, add a couple of drops of saline to extend its life. 7. Foundation can age you! Foundation isn’t older skin’s friend. It can settle into fine lines and accentuate wrin kles. It’s better to use concealer for younger looking skin. Opt for spot coverage. 8. You don’t need special creams and serums to reduce redness. Color correcting with green concealer is your best bet to remove redness from your skin! Using concealer with a gold hue could make you look younger as well. 9. Line your eyes with light color instead of black Lining your waterline, the rim of your eyes underneath the eyelashes, with a light color will open up your eyes. This is a great way to trick people into thinking your well rested. White or nude eyeliner will make you look more awake. Forget under-eye cream! 10. You don’t need a professional eye shadow palette Most expensive eye shadow palettes have tons of colors you’ll never use. Find the shades that suit you and buy them individually. Healthy Home, Healthy Environment Why Has Stormwater Runoff Become Such a Problem and How Can I reduce Runoff? By, Erin Mumby

Urbanization and increasing commercial and residential development have a great im pact on local water resources and our health. More impervious surfaces (roads, rooftops, parking lots and other hard surfaces that do not allow stormwater to soak into the ground) increase the rate of stormwater runoff. This means a greater volume of water carrying pollution into surface waters and less water soaking into the ground. These contaminants include litter; cigarette butts and other debris from sidewalks; motor oil poured into storm sewers; settled air pollutants; pet wastes; yard wastes; and pesticides and fertilizers from lawn care. Less water soaking into the ground also lowers ground water levels, which can dry up streams and hurt stream ecosystems, and can reduce the supply of well water.

Stormwater also erodes stream banks. This in turn degrades habitat for plant and animal life that depend on clear water. Sediment in water clogs the gills of fish and blocks light needed for subsurface plants. The sediment can also fill in stream channels, lakes and reservoirs, covering the bottom and negatively affecting flow, plants and aquatic life. There are inexpensive ways to control excess runoff created by patios, driveways, sidewalks and roofs. Whatever the soil drainage condition in your neighborhood, landscaping and careful grading of your property's surface area can be used to control runoff, reduce its speed and increase the time over which it is released. For example, land immediately adjacent to your house needs to have a downhill slope so that water does not seep through the foundation. Once the water has been carried 10 feet from the house, the surface should be graded so that runoff is released gradually. Surface runoff can be decreased, and ground water infiltration increased by following these suggestions: • Install gravel trenches along driveways or patios to collect stormwater and filter it into the soil. • Plant sod on bare patches in your lawn as soon as possible to avoid erosion. • Grade all areas away from your house at a gentle slope. • Use a grass swale, which is a man-made depression, to move water from one area to another. • Plant shrubs and trees to promote infiltration If you are building a new home or in a position to consider regarding your property, you may want to create a basin, which will hold all runoff and allow it to infiltrate the soil over a longer period of time. This should be done only where drainage is good. Alternatively, you may be able to create a gently rolling surface or a system of berms, or mounds, and swales to slow run-off. Berms and swales are slight elevations and depressions in the surface that provide channels along which water will flow. If you have a wet area, you may be able to move the basin to a less used area of the yard – around shrubs or trees, for example – by installing a swale to carry the water across the yard. Be advised that most activities performed in regulated wetlands require a permit. Contact DEP Land Use Regulation for information.
